One cell line, 349 drugs, and the meningioma evidence gap

IOMM-Lee is, for practical purposes, the malignant meningioma research program: the most widely used cell line for a tumor that lacks good models. A careful new characterization, with a 349-compound drug screen, sharpens both its value and the danger of leaning on it.

What the work claims

This is a primary characterization study, posted as a preprint and not yet peer-reviewed, from the University of Washington Department of Laboratory Medicine and Pathology1. The authors take IOMM-Lee, the most widely used cell line model of grade 3 malignant meningioma, and generate the reference data the field has been missing: whole-exome sequencing at 300X coverage, duplex-sequenced mitochondrial DNA variants, DNA methylation profiling, growth kinetics, radiation dose-response with and without an ATM kinase inhibitor, and a high-throughput screen of 349 anti-cancer compounds. The headline outputs are numbers the field will quote: a population doubling time of 18.9 to 20.7 hours, colony-forming efficiency of 87 percent when seeded at 25 cells per well, a radiation D10 (the dose leaving 10 percent clonogenic survival) of roughly 3.9 Gray, radiosensitization by the ATM inhibitor AZD-1390, and 34 distinct compounds that suppress proliferation at sub-micromolar IC50 with high drug sensitivity scores.

How it works

The genomic layer anchors the line in the modern meningioma taxonomy. IOMM-Lee is confirmed NF2-wildtype and carries a pathogenic TERT promoter variant (c.-124C>T). The authors are direct about what that means: the line is unusual among meningiomas, and especially among malignant ones, in lacking an NF2 mutation, while remaining clearly related to but distinct from other meningioma lines. In a tumor type where NF2 loss on chromosome 22 is the canonical founding event, the field's default model starts from a different genotype than most patients' tumors.

The growth data explain both the line's popularity and its bias. IOMM-Lee divides roughly every 20 hours and plates with up to 87 percent colony-forming efficiency, dropping to 69 percent when seeded from 200 cells per well; the authors note published doubling-time estimates vary nearly four-fold, which their standardized WST-1 assay resolves. Fast, efficient, reproducible: everything a screening program wants, and precisely the phenotype that maximizes sensitivity to drugs targeting proliferation and DNA metabolism.

The radiation experiments quantify radiosensitivity (D10 around 3.9 Gray, which is why the assay range was cut from 0-10 Gray to 0-3 Gray) and then radiosensitization: 1 micromolar AZD-1390, an ATM kinase inhibitor, given 1 to 3 hours before irradiation, radio-sterilized cultures at 5 Gray or more. A detail that belongs in every summary of this paper: 1 micromolar AZD-1390 alone, without radiation, suppressed the colony-forming efficiency of unirradiated cells by 23 percent, so part of the combination effect is single-agent activity, not pure sensitization.

The drug screen, run at the Quellos High-Throughput Screening Facility at UW, used a 349-compound commercial anti-cancer library. Nine agents had IC50 values under 10 nanomolar (counting one duplicate and two gemcitabine formulations), eight fell between 10 and 100 nanomolar, and eighteen between 100 and 1000 nanomolar, giving the 34 distinct sub-micromolar hits. The most potent class list reads like a catalog of antiproliferative mechanisms: the NAD biosynthesis inhibitor APO866 (also called FK866) at single-digit nanomolar, four mitotic kinesin or kinase inhibitors, taxol-family microtubule disassembly inhibitors, and gemcitabine formulations; the 10 to 100 nanomolar tier adds proteasome inhibitors, the 5-fluorouracil prodrug floxuridine, and methotrexate. Scoring through the FIMM Breeze 2.0 drug sensitivity resource, the authors report 15 of 17 non-redundant sub-100-nanomolar agents carried DSS2 scores above 20, their threshold for strong predicted utility.

Where a skeptic should push

The most load-bearing assumption is that response in IOMM-Lee predicts response in malignant meningioma patients. Every design choice that makes the line a good screening substrate also distances it from the disease. It is two-dimensional, so there is no extracellular matrix, no contact quiescence, no three-dimensional drug penetration gradient. It doubles every twenty hours, so a slow-cycling tumor cell with identical target expression could be entirely indifferent to compounds that win here; and the winning mechanisms, without exception, attack division or DNA handling. It is NF2-wildtype in an NF2-driven tumor family, and it carries a TERT promoter mutation whose contribution to any single hit is unknown. Nothing in the paper is tested in vivo, in a three-dimensional culture, or against a second malignant meningioma line, so "34 active drugs" is strictly a statement about one genotype in one growth regime. The IC50 values are also free-media values; serum protein binding alone can erase a hundred-fold margin in vivo.

To be fair to the authors, they do not claim clinical prediction. Their stated aim is narrower and defensible: to locate IOMM-Lee in the genomically defined meningioma landscape and to provide data types that let others use the line more intelligently. Read at that level the work is exactly what a field with few models needs. The risk is not in the paper; it is in the citation practice that will follow it, where "sensitive in IOMM-Lee" quietly becomes "promising in malignant meningioma."

The one-line trap for organoid meningioma models

For organoid-based drug discovery, this preprint is a blueprint wearing a warning label. The blueprint: the authors' data package is precisely the calibration sheet a patient-derived organoid program should demand before trusting its own screens. A meningioma organoid panel run against the same 349-compound library would give an instant, quantitative read on which hits are properties of the tissue type and which are artifacts of one fast line; the radiation and AZD-1390 arm supplies a ready-made positive control for three-dimensional radiosensitization assays, where drug penetration and hypoxia change the numbers. The growth-kinetics section is a standing reminder that even the most basic model parameters drift across labs, so any organoid-to-cell-line comparison needs its own anchored measurements.

The warning: meningioma is what happens to a therapeutic area when one model becomes the model. With essentially the entire preclinical pipeline for malignant meningioma running through a single NF2-wildtype line, there is no independent channel to catch a line-specific artifact before it reaches a regimen decision. Organoid programs can fill that channel, but only if they are built as replicates of the disease rather than three-dimensional replicas of IOMM-Lee: multiple patients, NF2-mutant and NF2-wildtype, graded by the same methylation and sequencing axes this paper uses. The threat to the organoid field is the lazy version, where aPDO panel is benchmarked to match IOMM-Lee's rankings and declared validated, converting the artifact into a standard. The opportunity is the adversarial version, where disagreement between line and organoids is the finding, because each disagreement marks a claim that was never about the tumor at all.

The bottom line

Established: IOMM-Lee is NF2-wildtype with a pathogenic TERT promoter variant, doubles every 19 to 21 hours, is comparatively radiosensitive with a D10 near 3.9 Gray, is radiosensitized by AZD-1390, and yields 34 sub-micromolar hits in a 349-compound screen. Hypothesis: any of those hits, or the combination strategy, will transfer to patients. What would confirm it: concordant activity in patient-derived organoids or xenografts spanning NF2 genotypes, and in vivo pharmacology. What would break it: organoid panels that fail the top hits while responding to drugs the line ignored, which would reclassify the screen as a measure of the line's growth rate rather than of the disease.

Frequently asked questions

What is IOMM-Lee?

IOMM-Lee is a grade 3 malignant meningioma cell line that the authors describe as the most widely used model of that tumor type. The new work confirms it is NF2-wildtype and carries a pathogenic TERT promoter variant.

Why does the NF2-wildtype genotype matter?

NF2 loss on chromosome 22 is the most common founding alteration in meningiomas. A model line that lacks it represents the minority of tumors, so findings in the line may not generalize to typical patients.

How was the drug screen done?

A 349-compound commercial anti-cancer library was tested at a University of Washington high-throughput screening facility. Compounds were ranked by IC50 and by DSS2 drug sensitivity scores from the FIMM Breeze 2.0 resource.

Which drugs were most potent against IOMM-Lee?

The sub-10-nanomolar tier included the NAD biosynthesis inhibitor APO866, four mitotic kinesin or kinase inhibitors, taxol-family microtubule inhibitors, and gemcitabine formulations. Most winning mechanisms attack cell division or DNA handling.

What did the AZD-1390 experiment show?

The ATM kinase inhibitor AZD-1390 at 1 micromolar, given 1 to 3 hours before irradiation, radio-sterilized the cells at 5 Gray or more. It also suppressed colony formation by 23 percent on its own, so the combination readout includes single-agent effect.

Is this peer-reviewed?

No. It is a bioRxiv preprint posted on 11 August 2026, which is why every number here should be treated as solidly reported but not yet independently reviewed, and not as clinical evidence.
